I will abide by the following guidelines as set forth by the prepares of this document.  I understand and agree to the following.

  The following will not be acceptable for use:

• Religious sites (unless related to curriculum)
• Adult magazines
• Adult Photos
• Adult chat Rooms of any kind.
• E-mail (sent or received).
• Hate sites
• Bulletin Boards
• User may not download without teacher permission.
• Use of the network in any manner that would disrupt the use of the network by others will not be tolerated.
• No publishing of materials on the Web.

The Internet is to be used for educational and research purposes only.

  The following consequences will be implemented upon discovery of faulty use.

Step One- Removal from computer for the day.

Step Two- Removal from the computer for thirty days and parent conference.

Step Three- Removal from computer for remainder of the year.

  The following will be permitted with written permission from teacher.

• Religious sites related to curriculum.
• E-mail projects

         "Name of Teacher"                                        denies any responsibility for the accuracy of information.  While I will make an effort to ensure proper use by the user.  The user has the ultimate responsibility for how the electronic resources are utilized and bear the risk of reliance of information a\obtained.

I acknowledge the school district has taken every precaution to ensure safe use of electronic resources and I further acknowledge that inappropriate information can be obtained if the user engages in aggressive pursuit of such information.

I have read and agreed to abide the school districts policy and regulations on appropriate use of electronic information systems as incorporated by reference. I understand and will abide by the provisions and conditions indicated.  I understand that any violations of the above terms and conditions may result in disciplinary action and revocation of my use of information services.

As parent or guardian of this student, I have read this agreement and understand it.  I understand that it is impossible for the school district to restrict access to all controversial materials, and I will not hold the district responsible for materials acquired by use of the information services.  I accept full responsibility for supervision if and when, my childís use of the information services is not in a school setting.  I herby give my permission to have my child use electronic information services.
